Domiciliary Care
Using a tailored approach, we design a care plan to suit service users in all forms of care be it emotional, physicals and many more.
We design a person centred dynamic care plan and work with the person’s wider circle e.g. client Doctor(s) & nurse(s), immediate families, close friends and relatives , solicitors and other professionals to ensure the plan remains effective and responsive at all times.
A regular record sheet is collated every visit / hour (depending on circumstances) to track service user responsiveness and invoke changes were needed. In short our dynamic care plan focuses on change control for the betterment of the service user.
